Support Coordination

Connect and Care Support Coordination aims to improve health and wellbeing outcomes for NDIS participants by helping them to participate more fully within their community. I do this by providing one on one virtual or phone service delivery throughout Australia regardless of location, making quality support and care more accessible.

For those people who live on the Central Coast, I can visit with you face to face in your home or in the community.

If support coordination is included in your NDIS plan, then you are already eligible to get assistance to not only understand the process, but to implement your plan the best way possible.

I can support you whether you are accessing the NDIS for the first time or if you have been a participant in the scheme for some time.

I do not discriminate and so support all Australians by offering people with disabilities guidance, and in the process helping them to learn new skills, building their capacity to become more independent and confident.

Support Connection/Training for Parents and Carers.

Support connection helps you build your abilities to connect with informal community and funded organisations | individuals | groups etc, which enables you and your loved ones to get the most out of your plan and pursue your life pathways.

This level of support assists you to build skill needed to understand and use your plan in the best way forward, ensure an increase in capacity, maintain relationships, manage service delivery tasks, live independently and find your place in the community.

Specialist Support Coordination

Specialist support coordination is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. As a specialist Support Coordinator, I can assist you to manage challenges in your support environment and by ensuring consistent delivery of service.

I work with people, their families, and carers to identify and understand the barriers impacting a person’s ability to access, use and maintain their supports.

It is my responsibility to find solutions to address those barriers and establish a process to overcome those barriers, including prioritising factors that need to be addressed first and what supports need to be put in place immediately.

Design a service plan for complex support needs, where appropriate, sometimes a participant may need a service plan to address significant complexities in their life.

Specialist support coordinators should work with the participant, their families, and carers to identify a broad network of supports that can help the participant use their plan.
